Patient Details:

Chief Complaint:
- Severe right ear pain since 2 days
 - Fever since yesterday
 - Irritability and crying at night
 
History of Present Illness:
- Child developed coryza with watery discharge 4 days back.
 - Pain started in right ear, worse at night, child cries and pulls ear.
 - Fever developed with restlessness and sleeplessness.
 - No ear discharge yet.
 
Past History:
- Recurrent colds and sore throat.
 - No history of TB, diabetes, or major illness.
 
Family History:
- Father: Allergic rhinitis
 - Mother: Hypertension
 
Physical Examination:
- General: Irritable, cries on touching ear.
 - Vitals: Pulse 110/min, Temp 101°F, Resp 24/min, BP 100/60 mmHg.
 - Ear Examination:
- Right tympanic membrane congested, bulging.
 - Tenderness on tragus pressure.
 - No discharge.
 
 - Throat: Congested tonsils.
 
Systemic Examination:
- RS: Clear, no added sound.
 - CVS: S1 S2 heard, no murmur.
 - Abdomen: Soft, non-tender.
 
Provisional Diagnosis:
Acute Otitis Media (Right ear)

- Chamomilla 200C, single dose, dissolved in water (for acute irritability, unbearable pain, child cries, wants to be carried).
 - Follow-up after 24 hrs.
 
Follow-up:
- Next day: Pain reduced, child slept better, fever subsided.
 - After 3 days: No pain, tympanic membrane less congested.
 - After 7 days: Complete recovery.
 
✅ Justification:
- Chamomilla is indicated in otitis media with severe intolerable pain, child irritable and wants to be carried, worse at night.
 - Belladonna could have been given if fever with marked congestion and throbbing pain predominated.
 - Pulsatilla for mild, clingy children with ear pain better in open air.
