2. Chief Complaints

  1. High-grade fever with chills – since 4 days
  2. Cough with thick, rusty sputum – since 3 days
  3. Chest pain on right side – since 3 days
  4. Shortness of breath – since 2 days

3. History of Present Complaints

Patient was apparently well 4 days ago when he developed fever with chills and rigors.
Fever was continuous, with marked weakness and body ache.

Next day, he developed dry cough, which later became productive with rusty-coloured sputum.
Complains of pain in right chest which is aggravated by coughing and deep breathing and ameliorated by lying on painful side.

He also feels shortness of breath, with a desire to open windows for fresh air.

4. Past History

  • Suffered from typhoid 6 years back
  • No history of tuberculosis, asthma, or diabetes

5. Family History

  • Father: hypertensive
  • Mother: healthy
  • No family history of tuberculosis or asthma

6. Personal History

  • Appetite: Decreased
  • Thirst: Increased
  • Desires: Cold water, sour food
  • Aversion: Milk
  • Bowel: Constipated
  • Urine: Dark yellow
  • Sleep: Disturbed due to cough and fever
  • Perspiration: Profuse during fever
  • Habits: Smokes occasionally (since 10 years)

7. Physical Generals

  • Thermal Reaction: Chilly
  • Build: Average
  • Weight: 65 kg
  • Height: 165 cm
  • Pulse: 104/min
  • BP: 120/78 mmHg
  • Temperature: 102.4°F
  • Respiration: 26/min
  • Tongue: Coated, dry
  • Complexion: Pale

8. Mental Generals

  • Anxious, restless, and fearful of serious illness
  • Irritable when disturbed
  • Desires to be in open air
  • Talks continuously during fever
  • Fears death at night

9. Physical Examination

Inspection:

  • Rapid breathing, flaring of alae nasi
  • Chest movement decreased on right side

Palpation:

  • Vocal fremitus increased on right lower lobe

Percussion:

  • Dull note on right lower lobe

Auscultation:

  • Bronchial breath sounds
  • Fine crepitations heard at right base
